How does lockout promote safety in the workplace?

CNESST Investigation
“On September 3, 2015, a worker finalizes the installation of a guide in the winding machine of a machine used to manufacture aluminum alloy rod. To perform the task, he accesses the top of the winder mandrel and crouches. A piece of equipment called the top pane is located at the top of it. Suddenly, the top shutter drops and wedges the worker against the chuck.
Consequence: the worker dies.
Causes: 1) The upper flap of A-winder starts to move and crushes the worker.
2) Repair work is carried out in A-winder while the equipment is energized.
3) Poor planning of the A-winder commissioning activities exposes workers to hazards. “
